Second tier providers support and subscribe to the ABI's GTA, but do not
meet full GTA requirements (eg do not have national coverage).
Protocol arrangements between individual insurers and second tier providers
are at the discretion of both parties who will also determine rates (which
should not exceed those of first tier providers). Listings indicate where
protocol arrangements have been set up.
